I, the undersigned parent or legal guardian, hereby give permission for my child to participate in programs, practices, camps, clinics, training sessions, and related activities operated by OKC Ambassadors (hereafter referred to as “the Organization”).
Assumption of Risk
I understand that participation in athletic activities, including but not limited to baseball instruction, training drills, conditioning, agility work, and use of athletic facilities and equipment, involves inherent risks. These risks include, but are not limited to, physical injury, illness, accidents, weather-related conditions, and other unforeseen events.
I knowingly and voluntarily assume all risks associated with my child’s participation, whether known or unknown, and acknowledge that injuries may occur despite reasonable care and supervision.
Release of Liability
In consideration of my child’s participation, I hereby release, waive, and discharge the Organization, its directors, officers, coaches, volunteers, staff, agents, and affiliates from any and all claims, demands, actions, or causes of action arising out of or related to my child’s participation, including but not limited to claims for personal injury, property damage, or wrongful death, whether caused by negligence or otherwise, to the fullest extent permitted by law.
Medical Authorization
I certify that my child is physically able to participate in athletic activities. In the event of an emergency, I authorize the Organization to obtain medical treatment for my child as deemed necessary. I understand that I am responsible for all medical expenses incurred.
Weather & Facilities
I understand that activities may be affected by weather, field conditions, or facility availability. Sessions may be modified, relocated, or canceled as necessary for safety reasons.
No Refund Policy
I acknowledge that registration fees are non-refundable, regardless of attendance, weather-related cancellations, illness, injury, or withdrawal from the program.
Photography & Media Release
I grant permission for the Organization to use photographs, video recordings, or other media of my child taken during participation for promotional, educational, or informational purposes, without compensation.
Code of Conduct
I agree to support the mission and standards of the Organization and understand that disruptive or unsafe behavior by a participant or family member may result in dismissal from the program without refund.
Acknowledgment
I have read and fully understand this waiver and release. I sign it voluntarily and acknowledge that it is binding upon myself, my child, and our heirs and legal representatives.
